Test Proctoring Procedure (3-02)

 

The SEARK Library and its staff will administer tests for SEARK faculty upon request .  The procedure outlined is designed for make-up tests for a limited number of students in a class.  Arrangements to have an exam proctored for an entire class must be made, in advance, with the Dean of the Library and Distance Learning Division.  Please contact the Dean of the Library and Distance Learning Division at 543-5988 or vperschbacher@seark.edu with any questions, problems or concerns.

 

Each test to be proctored must be accompanied by a “Proctoring Sheet  with section “a” completed by the instructor.  The exam and proctoring sheet should be handed to a full time library staff member.  Student workers will be instructed not to accept exams or tests to be proctored.  “Proctoring Sheet” forms are available in the library and/or may be copied as needed.

 

All test proctoring files and material will be kept behind the circulation desk in a drawer labeled “Test Proctoring Material.”  This file will contain folders for:

Regular test to be taken

Telecourse test to be taken

Regular test completed

Telecourse test completed

Completed Proctoring Sheets – current month

Proctoring supplies (blank Proctoring Sheets, scantrons, etc.)

 

When the student comes into the library to take her/his test, a full time permanent library staff member will administer the exam under the conditions set by the faculty member in section “a” of the “Proctoring Sheet.”  The library staff member will print the student’s name, name of test, date and beginning and ending times in section “b” of the “Proctoring Sheet.”  The student will sign where indicated.  After the test and section “b” of the “Proctoring Sheet” are completed, they will be filed in the appropriate folder (see above) until picked up by the faculty person.

 

Instructors must request completed exams from a full time permanent library staff member.  The faculty member will complete section “c” of the “Proctoring Sheet” which will be filed in the “Completed Proctoring Sheets” file by the library staff.

 

Completed “Proctoring Sheets” will remain in the “Completed Proctoring Sheets” file until the end of the calendar month, when they will be collected and counted for the Monthly Statistical report.  Completed Proctoring Sheets should be kept for one semester beyond the semester in which the exam was administered and then destroyed.

 

Non-administered tests will be returned to the instructor before the end of the semester, unless other instructions are given.
